Candidate Specification We are looking for an experienced leader who understands the needs of our clients and care team. The applicant needs to have good interpersonal skills and striving to achieve a high standard of care.

Candidate Qualification & Key Skills

  • The candidate must have previous 2-year domiciliary care. coordinating experience.
  • The candidate also must own his/his/their own vehicle and be able to drive.
  • the candidate must have strong communication skills both written and verbal and be able to liaise with health professionals such as social workers, OT’s and Physiotherapists, competently.

Candidate Key Accountabilities

  • Assisting the Registered Manager with managing care staff.
  • Shared on-call duties.
  • Producing care rotas and managing staff through roster software Birdie.
  • To receive and process new referrals before care commences.
  • Ensure that where needs assessments are carried out, a support personalised plan is created.
  • Accepting care packages and assigning cares to service users in an appropriate manner.
  • Liaising with clients, local authorities and other stakeholders as part of a multi-agency approach.
  • Arranging care reviews and risk assessments for service users.
  • Participating in the branch on-call rota.
  • Ensuring that our service complies with CQC requirements.
  • Working on the rounds to cover  weekdays, evenings, weekends if  there are gaps in the carer rota.
  • Recruitment, Induction, supervision, and Appraisal of staff.
  • Delegating tasks to care staff and following up to make sure tasks are completed to a high standard.

JOB TITLE: SUPPORT WORKER

JOB DESCRIPTION:

Working as a member of the care team in the provision of care and support services for the elderly clients suffering with dementia or general complex ill health in their own homes. Clean driving licence and use of own car is essential for this role.

MAIN RESPONSIBILITIES  

  • Helping or supervising client washing, dressing, grooming, and toileting
  • Preparing client meals and snacks and feeding or supervising eating and drinking
  • Administering medications
  • Cleaning home, washing clothes and linens, shopping and other domestic duties as required
  • Monitoring client health conditions
  • Using Company’s reporting system to record information from each client visit
  • Reporting any concerns to Care Co-ordinator
  •  

SKILLS / ABILITIES

  • Knowledge and understanding of challenges relating to people with dementia
  • A commitment to a service which provides support to vulnerable individuals, and which is respectful and according to the individuals’ wishes
  • Good Level of English (written and verbal)
  • Good communication skills. Ability to communicate effectively with clients, colleagues, managers and other care professionals.

EXPERIENCE

  • Experience of working in the care sector preferred

EDUCATION

GCSE English Level 4 or Equivalent

OTHER REQUIREMENTS

  • Clean driving licence and use of own car for business purposes
  • Willingness to undertake training including Care certificate and Client specific training
